Application Forms - Graduates & NCLEX Examinations

There are two methods for an applicant to apply to the Texas Board of Nursing (BON) to take the NCLEX-PN® and NCLEX-RN® through the State of Texas: 1) Online or 2) Paper.

Note: The Nursing Jurisprudence Examination (NJE) and Pearson Vue (PV)* registration MUST be completed BEFORE you can receive your Authorization to Test (ATT) and in turn your GN/GVN permit (if applicable).

*If you HAVE registered with Pearson Vue, but it is not showing a date of completion, first verify with PV that you selected Texas as your testing jurisdiction. If your jurisdiction is correct, then email webmaster@bon.texas.gov with the subject line “PV Payment Update to File” and include your full name, DOB, and date you paid PV in the body of your email. This will be updated within ten (10) business days.

School Code:

The online application will request the entry of the ten (10) digit code representing your Basic Nursing School. This code can be obtained from your school of Nursing or from one of the MS Excel files listed below:
